The full picture
What happened
Boeing 737-500 operated flight Moscow — Perm. At night, the crew received permission to approach, but the trajectory and altitude required additional maneuvering. After stopping the first approach, the aircraft had to gain altitude and return to the scheme.
During the reversal, there were uncoordinated actions to control thrust and roll. The spatial position began to deteriorate rapidly, the aircraft went into steep decline and collided with the ground in the city near the railway.
The commission attributed the loss of control to the commander’s disorientation and pointed to a whole background of systemic factors: insufficient training, weak interaction in the crew, fatigue and alcohol found in the commander’s body. All 88 people were killed.
Context and mechanisms
Detailed account
Background, technical details and decisions are examined separately to avoid reducing a complex event to a single explanation.
Night, Clouds and Growing Workload
Flight 821 took off from Moscow Sheremetyevo to Perm on a Boeing 737-505 VP-BKO. There were 82 passengers and six crew on board. The flight to the destination area took place without failure, which would make the aircraft uncontrollable. The critical chain was formed at the final stage - at night, in the clouds, when there was no external horizon and the crew depended on the instruments.
Approach to the airfield demanded to change the original scheme and extinguish the height. Negotiations, navigation and speed control took place simultaneously. The aircraft did not come out on a simple, pre-stabilized profile, so each new dispatcher command was added to the already high load. In such a situation, it is especially important to distribute the roles in advance: one pilot controls, the second controls the parameters and communication.
The Commission has not established a failure of the airhorizon or a control system that would explain the coup. The autopilot and traction machine in the final section were disabled, and the commander piloted manually. This is not a violation in itself, but sharply increases the requirements for the skill of reading a direct indication of the spatial position and for continuous control of the co-pilot.
Before the transition to Boeing, both pilots had experience on types with a different air range logic. On modern Western instruments, the symbol of the aircraft remains relatively still, and the horizon line moves. With fatigue and stress, the brain is able to return to the old motor habit and misinterpret the direction of the roll, especially if the training on the new type was not deep enough.
Different thrust and serviceable controlled aircraft
On the plane, there was a so-called fork of the position of the engine controls: to obtain close thrust, the levers had to be held asymmetrically. This defect has not been corrected for a long time by the technical service, although the operational documentation set permissible limits and actions. During the approach, the difference increased the number of minor corrections and distracted the attention of the crew.
It is important not to turn the RUD fork into an independent cause of the disaster. Engines continued to create thrust, the aircraft responded to controls, and simulations confirmed the possibility of safe flight. The defect acted as a load amplifier: at the time when attention was required for the spatial position, the pilot received additional asymmetry of readings and sensations.
After stopping the approach, the crew had to set a set of altitude, withstand the course, control the thrust and make a turn. On an automated aircraft, the usual barrier could be an autopilot, but the final maneuver was performed manually. When the roll began to grow, the commander gave control effects that did not stop, but developed a deviation.
It was not about a hidden technical loss of control, but about controlled flight, which turned into a loss of control due to an incorrect perception of the situation. This is a fundamental difference: the serviceability of mechanisms does not guarantee security if a person receives the correct data, but interprets them through an erroneous internal sense of the horizon.
Disorientation does not arise in emptiness.
The human vestibular apparatus is not designed to reliably determine the position in the clouds. After a few seconds of even roll, the feeling of rotation disappears, and when trying to level the plane, an illusion of roll in the opposite direction may occur. The only reliable support is devices and their systematic cross-reading. It is this skill that should be brought to automatism.
The Commission found a lack of training in manual piloting, CRM and deriving from a difficult spatial position. Formal qualifications did not compensate for gaps in practical skills. Pilots could perform the usual operations, but were not ready for quick recognition of an unusual position with disconnected automation and high load.
The study of the mode of work showed the accumulation of fatigue in the commander and the discrepancy between rest and standards. Forensic examination revealed ethyl alcohol in his body. These circumstances cannot be used for sensation or psychological portrait of the deceased; their significance is technical: they impair attention, the speed of instrument information processing and the quality of decisions.
The co-pilot did not create an independent safety circuit. CRM requires naming a specific deviation - roll, pitch, speed - and amplifying the intervention if the commander does not respond. At the last stage, a polite remark is not enough: in case of a threat of loss of control, the second pilot should clearly demand a correction or take control. Organizational culture should give it such a right in advance.
Disaster as a result of a weak admission system
The IAC called the systemic reason for the insufficient level of organization of flight and technical operation of the Boeing 737 in the airline. This wording is broader than the actions of two pilots. It covers selection, retraining, skills testing, documentation control, defect elimination, fatigue management, and management’s ability to detect dangerous trends before a flight.
The transition from Soviet types to Boeing required a proven possession of the new indication, and not only the raid and the surrender of the theory. The simulator had to reproduce the care for the second round at night, manual reversal, failure or shutdown of automation and exit from large rolls. If the test evaluates only the standard setting, the most dangerous skill deficit remains hidden.
Technical service is also part of flight safety. Unremoved RUD fork did not destroy the aircraft, but showed that a known discrepancy could persist in operation. In a mature system, the recurring defect is tracked, assessed for risk and eliminated, and information about it is transmitted to the crew without turning the abnormal state into a familiar background.
After the disaster, the findings were discussed not as a criminal definition of guilt, but as a prevention material. The practical result is increased preparation for manual flight on devices, direct indication of the aircraft horizon, upset recovery and CRM. The main idea is simple: a difficult step should not depend on whether one tired person will be able to recognize his own illusion alone.
Sequence
Event timeline
Rather than isolated episodes, this was an unfolding system: each stage inherited the constraints of the one before it.
- 01
Complex geometry of the approach
The aircraft approached the airfield with parameters that required correcting the course and altitude. Work load increased at night instrument conditions.
- 02
The trajectory is not stable.
The crew did not form a stable profile. The decision to continue maneuvering left less time to control the basic parameters.
- 03
New Set of Commands
After stopping the approach, it was necessary to simultaneously control the thrust, pitch, roll, communication and navigation. It is the transitional regimes that are particularly sensitive to the weak distribution of tasks.
- 04
Spatial Disorientation
The commander perceived the position of the aircraft not as it was shown devices. Management actions strengthened the roll instead of eliminating it.
- 05
Cool Decline
The roll has reached a great value, the vertical speed has grown rapidly. There was no time or height left for recognition.
- 06
Collision with the Earth
The plane crashed into Perm and completely collapsed. There were no dead on the ground.
More than one cause
Chain of contributing factors
An incident develops when several weaknesses come together along the same path.
Training
The Commission identified gaps in the skills of manual piloting, working with Western avionics and restoring the spatial position.
CRM
The co-pilot did not form effective mutual control and did not stop the development of the dangerous regime with decisive intervention.
Physiological background
Fatigue reduces the speed of recognition, and the presence of alcohol is incompatible with safe flight performance. These circumstances reinforced each other.
Organization
The system of admission, training and control did not filter out the risks before they met in one crew and one difficult approach.
From the flight deck
Decisions and constraints
Decisions are assessed against the information, altitude, time and aircraft condition at that moment, rather than what became known later.
Why devices could contradict the sensations
Without an external horizon, the vestibular apparatus easily creates a false sense of straight flight. A safe strategy is to trust the air horizon and cross-confirm the parameters.
Role of the co-pilot
CRM requires not only to report a deviation, but to increase persistence up to the interception of control if the aircraft goes beyond safe limits.
Stable entry
Leaving for the second round is a normal protective action. But he himself must be carried out according to a clear procedure, with pre-distributed responsibilities.
Summary
Investigation findings
- A technical failure, which in itself would explain the disaster, has not been established.
- The immediate mechanism was the loss of spatial orientation and control during the reversal after the aborted approach.
- Crew training and flight work did not provide the necessary safety barriers.
- The difference in the position of the traction levers increased the workload, but was not the only reason for the loss of control.
After the investigation
What aviation learned
A lesson becomes useful when it leads to a procedure, training, a design change or a new safety barrier.
Practical training
The focus is on manual piloting by devices, getting out of a difficult spatial position and working with the failure or shutdown of automation.
Control of admission
The operator needs a traceable assessment of qualifications, language training, working conditions and medical restrictions.
Strong CRM
The hierarchy in the cockpit should not prevent the co-pilot from stopping a dangerous decision or taking control.
Its relevance today
The aircraft does not equal the stability of the system
The Permian disaster reminds us that loss of control can happen without destroying the part. Devices, engines and controls are capable of working, and the combination of night, clouds, weak skills and an incorrect sense of the horizon still leads to a coup. Therefore, the verification of equipment does not replace the verification of competencies and working conditions.
For airlines, this is an example of why training cannot be considered completed after the issuance of a permit. We need an analysis of real flights, repeated deviations, the quality of manual piloting and the behavior of the crews when leaving for the second round. Technical defects, such as forks, should also not become a familiar feature of a particular side.
For the crew, the main barrier is the joint reading of one reality. The co-pilot is not an observer, but an independent error detection channel. A clear report of the angle of heel, correction teams and readiness to intercept the control must be worked out in advance, so that at a critical moment it does not depend on personal courage.
A separate lesson relates to risk management before closing doors. The schedule of rest, medical fitness, confirmed experience on the type, the quality of re-training and the condition of a particular board usually belong to different units. If everyone sees only their acceptable fragment, the dangerous combination may go unnoticed. Independent supervision is needed precisely to collect these weak signals into the overall picture before the flight, and not to find one culprit after the disaster.
